Nomination Form
Physical
Therapist Assistant Award is intended to recognize
outstanding clinical performance and contributions to the
development of physical therapist assistants through VPTA
participation. This award was proposed by the VPTA Physical
Therapist Assistant Special Interest Group (PTASIG) and the
first award was presented to a physical therapist assistant
at the VPTA Annual Conference in April, 1992. |
| Purpose: |
To
recognize physical therapist assistants who have made an outstanding
contribution to clinical practice and to the physical therapist
assistants' development through participation in professional
association activities. |
Prerequisite
Criteria: |
The
nominee must:
· be a physical therapist assistant member of the VPTA
and a district counterpart
· have held elected office or committee participation
at the District or State level
· not be a member of the VPTA Nominating Committee

| Supportive
Narrative |
Two supportive narratives are required for each nominee. The
narrative should emphasize the nominee's contributions to
the profession in areas of leadership, clinical education,
and/or administration. Specific examples are recommended to
assist the Nominating Committee in the evaluation process.
Expertise and commitment toward improving clinical skills,
level of competence, and clinical education are essential
components. |
Selection
Process: |
Nominations
must be submitted to the Nominating Committee Chair at least
2 (two) months prior to the VPTA Annual Chapter Conference.
The Nominating Committee will make the final selection with
the knowledge of the VPTA President. |
| Presentation: |
The recipient will receive an engraved plaque and a $250 cash
award, which will be presented at the VPTA Annual Chapter
Conference by a PTASIG representative. The Physical Therapist
Assistant Special Interest Group funds this award.
